Anyone over the age of 18 (coaches, trainers, parents, admin, bus drivers, etc.) will need to complete the link below. Any players on the high school team that are 18 or over do not need to complete the link, (ie. Senior athletes). NOTE: Those with a valid DoD ID do not need to fill out the visitor form.
